If you, or anyone at your home or business premises depends on life support medical equipment (or to put this another way, are ‘life support dependent’), this needs to be registered on your account.
What is life support?
You are classified as being life support dependent if you depend on any of the following equipment:
- Oxygen concentrator
- Intermittent peritoneal dialysis machine
- Kidney dialysis machine
- Chronic positive airways pressure respirator
- Crigler najjar syndrome phototherapy equipment
- Ventilator for life support
- Any other equipment (whether fuelled by electricity or gas) that a registered medical practitioner certifies is required for life support, or a medical condition which requires continued supply of gas.
How do I register?
Please contact our team on 1300 608 387 or submit an enquiry online.
A Medical Confirmation Form will be sent to you, including a letter with practical advice of what to do in the event of a power failure or interruption (planned or unplanned).
Please complete all sections of the Medical Confirmation Form accurately, as incomplete information may cause delay. If needed, your doctor/GP can assist you with filling out the form. Remember to have both yourself and your doctor/GP sign the form.
The completed form can be sent to us either by email or post:
- Email: hello@seene.com.au
- Post: Locked Bag 14060 Melbourne VIC 8001
If you require more time to complete the Medical Confirmation form, please contact us to arrange a once-off extension.
What is seene's registration process?
- We’ll advise your local energy distributor and seene's energy provider to make sure you receive extra protections with your power supply.
- Life support status will be temporarily added to your account, and it will be permanently added once a completed Medical Confirmation Form has been received.
